Vaccination certificate

A vaccination card or vaccination passport is a health certificate proving vaccination that a person or animal has received against certain diseases with a certain vaccine.
For this document, the paper form is the standard. In the course of the coronavirus pandemic, Regulation (EU) 2021/953 has been adopted in the Member States of the European Union with effect from 1. In addition, the free issuance, verification and cross-border recognition of digital COVID certificates was introduced in July 2021.
International vaccination certificate
The international vaccination or prophylaxis certificate () is a health document by type. 36 of International Health Regulations. A traveller holding such a vaccination certificate shall not be refused entry because of the disease to which the certificate relates. Appendix 6 to the IHR contains a model certificate from which no derogation may be made. The certificate is valid only if the vaccine or prophylaxis used has been approved by the World Health Organization (WHO). Other health documents may not be required in international traffic (Art. 35 IHR), except in the case of signs of danger to public health. In this case, the competent authority may request travellers to complete forms with contact information and questionnaires about the health of travellers (Art. 35 sentence 3, Art. 23 IHR).
Yellow fever is the disease specifically identified in IHR Annex 7, for which, according to the IHR, proof of vaccination or prophylaxis may be required by travellers as a condition of entry into a Contracting State. Yellow fever vaccination is subject to entry requirements according to the annually updated WHO list of countries.
The Federal Ministry of Health is published in Art. 4, par. 1 sentence 2 no. 5 of the German law on the International Health Regulations, which entered into force with effect from 28 July 2007, authorizes statutory regulations necessary for the implementation of the IGV on the obligation of travellers to submit health documents (Article 35, 36 IGV). Such a legal ordinance may restrict the fundamental right of free movement (para 2 GG).

Vaccination documentation
Vaccinations for protection against a communicable disease (No. 9 IfSG) must be documented immediately by the person entitled to carry out the vaccination, usually a doctor (Para 1 IfSG).
Content
The vaccination documentation must be compliant with m. Para 2 IfSG contain the following information:
date of the preventive vaccination;
the name and batch designation of the vaccine;
name of the disease against which vaccination has been carried out,
the name of the vaccinated person, date of birth and name and address of the person responsible for carrying out the preventive vaccination; and
Confirmation in writing or in electronic form with qualified electronic
